Outreach Co-Ordinator, Acquired Brain Injury
Fixed term, 6 months, 30 hours per week
Location:
Galway
Job Purpose:
The Outreach Key Worker will be responsible for assessing and supporting clients with an ABI to achieve their desired outcomes. Working as part of a multidisciplinary team the successful candidate will be responsible for provision of person centre services and to support each client to identify progression routes to training, volunteering, social, leisure, vocational and educational opportunities.
Delivery of supports is fostered in a flexible manner, through a variety of activities in response to the identified needs of each participant. The successful candidate will also contribute to the continuous improvement activities of National Learning Network and promote a culture of excellence throughout the organisation.
This is a challenging role and the ideal applicant will have an appropriate professional qualification, or equivalent, together with a number of years relevant work experience. They will require excellent facilitation, communication and organizational skills coupled with an ability to develop and implement training modules on an individual basis.
Minimum Education & Experience required:
An appropriate professional qualification, or equivalent, together with a number of years relevant work experience.
A full clean driver’s license is required.
Job Duties & Responsibilities
- Conduct a detailed assessment of the each client’s needs and working as part of a multi disciplinary team develop a plan to meet the person’s needs, goals and aspirations.
- Providing rehabilitative supports and information in line with the individuals goals and objectives.
- Maintaining all records required for effective monitoring of the clients progress.
- Liaise with the Quest centre manager and Quest multidisciplinary team and other instructors in other NLN centres
