Community Support Worker - Neuro Rehabilitation
Permanent Contract for 31 hours per week
Location: Acquired Brain Injury service, Galway City.
Job Purpose
The Community Support Worker will assist the Community Services Manager and Team Leader in the delivery of a quality individual focused service. They will work individually and as part of a team providing high quality care and rehabilitative support to adults with Acquired Brain Injury and/or other neurological conditions in the community in a person centred service. They will be responsible for implementing and ensuring RehabCare standards while overseeing the delivery of a quality, customer focused service.
What we’re looking for:
QQI Level 5 in Health & Social Care or equivalent at a minimum, level 6 desirable.
Experience in the area of supporting individuals with intellectual, physical and mental health challenges.
Experience supporting people with ABI and/or neurological conditions.
Willingness to participate in RehabCare mandatory and further development training programme.
2 years’ experience is desirable in the provision of community and service based programmes and activities to promote independence and meet the needs, choices and abilities of individuals.
Experience of facilitation, implementation and review of Support/Care Plans.
Specific duties & responsibilities
To support service users living in supported accommodation and in their own homes to carry out rehabilitation programmes for physical, cognitive and emotional impairment to maximize their quality of life.
To help implement short term person led, rehabilitation goals as part of a short term neuro rehabilitation programme offered by the service to those living with a neurological condition.
To liaise with families, multidisciplinary teams and other personnel, as directed by the management team, to ensure the best quality of service with the individuals as directed by community services manager.
To carry out any other duties requested by Senior Management, which are within the scope of the post
To contribute to the overall development of the service and promote a positive image of the service users and RehabCare services.
To maintain a high delivery of service in line with Health and Safety standards and regulations
To adhere to and ensure the required standards in line with HIQA regulations and reporting to HIQA is maintained
Excellent communication, interpersonal, organizational, logistical and IT skills
To document information and complete paperwork as deemed necessary for delivery of service to the individuals
To support the individual with all aspects of activities of daily living such as potentially helping with personal care, budgeting , grocery and personal shopping, planning and booking of appointments, record keeping and communication between the individual and all other agencies that may be involved in the wrap around care for the person.
Additional Requirements:
- A flexible shift system will operate

- A full, clean driving license
Salary starting from point 1 €29,374.70 to point 5 €33,489.25 (pro rata, based on a 39-hour full-time work week), is subject to experience and qualification.
